We are transitioning to an electronic system for consent forms to adhere to government regulations. Initially, you may receive a consent form via email before your appointment or be provided with one during your visit. Feel free to fill it out at home beforehand or bring a smartphone to complete it just before your tattoo appointment. In the future, an automated system will send out consent forms along with your confirmation message.
