Note: We're in the process of moving over to an electronic system for consent forms  (see above link to complete form)  to comply with government guidelines. This is a new process for us and we will eventually have an automated system that will send out consent forms with your text confirmation message. So in the meantime, a consent form may be emailed to you ahead of your appointment or given to you at your appointment so please feel free to fill it in at home before you come in, or bring a smartphone with you and you can fill it out just before getting tattooed on your own phone.

Please be informed we will not accept COVID excuses for reschedules on day of appointment, deposit will forfeit if canceling and or rescheduling on day of your appointment unless you have confirmation of required self isolation.
